FIELD: medicine.
SUBSTANCE: with the aortic side-biting clamping "end-to-side" type an anastomosis of the shifting synthetic vascular prosthesis with the ascending aorta is formed. After that, the brachiocephalic branches are subjected to prosthetics with additional prostheses, sewn into the main shifting prosthesis. After starting the blood flow, three radio-opaque clips are applied: one on the aortic adventitia on the proximal edge of the anastomosis of the shifting prosthesis with the ascending aorta, two other ones - at the level of the distal edge of the anastomosis of the shifting prosthesis with the aorta on the adventitia of the ascending aorta on the upper and lower wall. After that layer-by layer- wound suturing is performed. Then, the stent is passed in such a way as to superpose the radio-opaque marl on the proximal part of the graft and the clip, applied on the aortic adventitia near the orifice of the shifting prosthesis. The stent-graft is opened in such a way that the radio-opaque marks do not displace with respect to each other. Control aortography is performed to estimate the stent opening and the presence of flowing, and if necessary additional unfolding of the implanted stent with a balloon is carried out.
EFFECT: method makes it possible to save the patient from additional irradiation due to the absence of installation and intermediate radio-opaque procedures and carrying out only control radio-opaque examination in the end of interference, reduce a dose of an introduced contrast substance and reduce the interference time.
