SUBSTANCE: bone “window” of the nasolacrimal fistula is formed on the lateral wall of the nasal cavity with a sonotrode for piezosurgical operations of the “Rasp” type of SONOCA 185 model of piezoelectric Soring apparatus with an irrigation system connected to it for cooling. The dissection of the bone mass is carried out under the following conditions: the mode of supplying ultrasonic energy to the sonorod of the “Rasp” type of the piezoelectric Soring apparatus model SONOCA 185 is pulsed, the oscillation frequency is 35 kHz, the power of the ultrasound used is 40%, the volume of the irrigation flow is 60%, the temperature of the saline solution supplied through the irrigation channel of sonotrode is 5°C, simultaneous exposure of bone structures to ultrasound is 5 seconds.

EFFECT: method allows to reduce the amount of damage in the surgical treatment of chronic dacryocystitis and the accompanying obstruction of the nasolacrimal duct, reduce the risk of intra- and postoperative complications and relapses due to low traumatization of the tissues and vessels surrounding the intervention area and a less pronounced inflammatory reaction of the surgical wound.
