FIELD: medicine; cardiovascular surgery.

SUBSTANCE: aortic arch prosthesis is performed using the “elephant trunk” technique with immersion of the prosthesis into the descending aorta, cutting out a 3 cm wide ring from the linear aortic prosthesis, wherein the diameter of the prosthesis is selected based on the size of the unchanged aorta, and the formation of a distal anastomosis between the arch prosthesis and the descending aorta. The cut ring is dissected, the aorta is bypassed with a dissector 3 cm distal to the formed anastomosis with the prosthesis, the ring from the prosthesis is passed under the aorta, fixed with formation of an annular cuff over the aorta and the prosthesis using a single-row continuous polypropylene suture with the mandatory alternating stitching of the synthetic linear prosthesis, the aorta and the cuff ring.

EFFECT: method makes it possible to strengthen the distal anastomosis when replacing the aortic arch using the “elephant trunk” technique, which leads to reliable sealing of the anastomosis, strengthening the aortic wall, modelling the aortic diameter to match the diameter of the prosthesis, reducing the load on the anastomosis suture area, does not require additional materials and equipment, namely a hybrid prosthesis, haemostatic agents and glue, as well as additional blood components and is technically feasible in any department of cardiovascular surgery.
