SUBSTANCE: diagnostic laparoscopy is performed through a trocar installed in the paraumbilical area. If an adhesive process is detected, adheolysis is performed. In the absence of adhesions or after adheolysis, 4 additional ports are installed: two 5 mm trocars in the right and left iliac regions, two 10 mm trocars between the umbilical and right and left iliac regions along the lateral edges of the rectus abdominis muscles. After this, the involvement of the pelvic organs and blood vessels in the pathological process is assessed: common and external iliac arteries, and veins. Then the pathological organ complex is mobilized: the pelvic organs involved in the pathological process. If the above blood vessels are involved in the pathological process, a laparotomy is performed and the affected areas of the vessels are resected with their further prosthetics, after which the pathological organ complex is exenterated. If the common and external iliac arteries and veins are intact, laparoscopic removal of the pathological organ complex along with lymphatic tissue is performed as a single block, after which the reconstructive stage of the operation is carried out.

EFFECT: method makes it possible to carry out surgical procedures in a certain sequence, to ensure adequate visualization of the pelvic organs by placing trocars of smaller diameter at certain points, to reduce blood loss, to reduce the frequency of early postoperative complications, to reduce pain, and to shorten the time of hospital treatment.
