SUBSTANCE: invention can be used to perform surgical treatment of patients with lumbar intervertebral hernias. When a patient is diagnosed with stage IV intervertebral disc degeneration according to Pfirrmann, extrusion or sequestered type of intervertebral hernia, the presence of changes in the red bone marrow and endplates of the adjacent vertebral bodies according to the Modic 0 type, and the range of motion in the spinal motion segment less than 10°, surgical treatment of lumbar intervertebral hernia is carried out, during which an incision is made in the skin and subcutaneous fat in the projection of the affected spinal motion segment. Along the lateral edge of the spinous processes from the side of the hernia, the tendons of the attached muscles are cut off, the muscles are displaced laterally, the interstitial space is mobilized, the ligamentum flavum is removed, the lower edge of the cranial vertebra and the upper edge of the caudal vertebra are resected. The hernial protrusion, which compresses the spinal root in the spinal canal, is removed. Using a scalpel, a window is made in the area of the fibrous ring defect commensurate with the base of the hernial protrusion, and the sequestered parts of the nucleus pulposus are removed from the intervertebral space. A visual inspection of the position of the dural sac and spinal roots is performed. The wound is washed with saline solution and the wounds are sutured layer by layer.
EFFECT: method eliminates the risk of compression of the nerve roots along with the accompanying artery, and restores lumbar lordosis by relieving the antalgic component.
