FIELD: medicine; otorhinolaryngology; maxillofacial surgery.

SUBSTANCE: skin incisions are made along the upper edge of the eyebrows, starting 5 mm medial from the supraorbital notch, towards the bridge of the nose up to 40 mm long. Then, from both sides towards the bridge of the nose and the area of the fracture, the underlying tissues are bluntly peeled off with a rasp, creating a common wound cavity. An inspection is carried out for the presence of foreign bodies. Endoscopic endonasal expansion of the nasofrontal canals is performed with the installation of drains. Through one of the percutaneous superciliary approaches, a titanium mesh is inserted into the wound. Through another access, a clamp is used to capture the titanium mesh. After this, the titanium mesh is placed on the defect area so that its edges protrude at least 5 mm beyond the boundaries of the defect. The titanium mesh is fixed with screws using a bur to the healthy bone tissue of the frontal bone.

EFFECT: reduction of the invasiveness of the intervention during the surgical treatment of post-traumatic extensive comminuted fracture of the anterior wall of the frontal sinus, and elimination of extensive disturbances of skin sensitivity in the postoperative period.
