FIELD: medicine; endoscopy; gastroenterology.
SUBSTANCE: marking is performed in the form of a “three-leaf clover” in the area of the greater curvature, the anterior and posterior walls of the stomach. Marking of the intended area of resection of the mucous membrane of the cardial part of the stomach is performed. An injection is made along the edge of the marking with saline solution with the addition of indigo carmine dye using a 25-gauge needle with a 4 mm tip. A fragmentary resection of the gastric mucosa is performed using a VIO300D ERBE installation in the Endocut Q mode.
EFFECT: possibility to form a valve of the mucous membrane on the side of the greater curvature, which forms an acute angle of His due to cicatricial narrowing on the side of the mucous membrane, thereby pulling the fundus of the stomach to the oesophagus, reducing the risk of developing complications in the postoperative period associated with scar strictures, as well as reducing the risk of relapse, shortens the rehabilitation period.
