METHOD FOR SURGICAL MANAGEMENT OF IDIOPATHIC THORACIC SCOLIOSIS OF IV DEGREE BY SELECTIVE HYBRID FIXATION Russian patent published in 2024 - IPC A61B17/70

Abstract RU 2816028 C1

FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to traumatology, orthopaedics and vertebrology, and can be used for surgical treatment of idiopathic thoracic scoliosis stage IV by selective hybrid fixation. Skin is incised in a projection of the spinous processes of the vertebrae along the entire length of the spinal deformity, and the subcutaneous fat and superficial fascia are dissected. Soft tissues are separated with a coagulator until the base of spinous processes, vertebral arches and facet joints are exposed on both sides. Transpedicular screws are inserted into the vertebral pedicles in the thoracic spine in the proximal and distal direction from the apex of the deformation, insertion of LSZ hooks under the vertebral arch on the apex of the deformation from the concave side, insertion of the rods into the heads of the transpedicular screws. Auto-bone crumbs are laid. Two active drains are placed in the metal structure bed in the caudal and cranial edges of the incisional wound. Wound is closed in layers. Transpedicular screws are inserted as follows: at the ends of the deformation of the convex side, two pairs of screws are installed in a row on the adjacent vertebrae, the following screws are installed through one vertebra, from the concave side of the deformation, the screws are installed through one vertebra.

EFFECT: method provides correction of spinal deformation and reduction of surgical aggression during surgical intervention due to installation of anchor points of fixation on apex of deformation from the concave side in the form of LSZ hooks, in combination with transpedicular screws above and below the deformation apex.
