FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to otorhinolaryngology. Behind-the-ear incision, an antromastoidotomy, a posterior tympanotomy are performed. When performing a posterior tympanotomy, a posterior superior and a posterior inferior tympanotomy are formed so that a bone bridge is preserved between them. Posterior superior tympanotomy is limited by a bone bridge over a posterior ligament of incus, a chorda tympani, a mastoid segment of the facial nerve canal and a bone bridge. Posterior inferior tympanotomy is limited by a bone bridge, a chorda tympani, a mastoid segment of the facial nerve canal and a bone mass within the origin of the chorda tympani from the facial nerve canal. Tympanic cavity is then filled with dexamethasone so that the cochlear opening is covered with the solution. Further, the implant electrode is delivered through the antromastoid cavity, the posterior inferior tympanotomy into the tympanic cavity. Thereafter, the cochlear window membrane is opened through dexamethasone solution. Further, an electrode is inserted into the lumen of the cochlear spiral canal up to the proximal ring of the electrode of the implant. Silicone tube is then inserted through the posterior superior tympanotomy into the tympanic cavity to the hypotympanum. Postoperative local glucocorticosteroid therapy by intratympanic administration of dexamethasone solution is carried out through a silicone tube.
EFFECT: method allows to preserve residual auditory function in patients during cochlear implantation, to reduce local inflammatory reactions in cochlea.
