FIELD: medicine.

SUBSTANCE: invention relates to abdominal surgery, coloproctology. First stage involves resection of a portion of the ileum with preservation of its mesentery. J-shaped reservoir is created from the produced autograft. Anastomosis is formed between an ileal reservoir and a rectal stump. Proximal ileal stump is closed and anchored to the colostomy. In 10–14 days, rectum and autograft are hydrotrained. At the second stage, a colostoma is excised. Anastomosis between a colon and a proximal ileal stump is formed through a parastomal approach.

EFFECT: method enables to perform reconstructive operations in patients with large resections of the left colon in the presence of a short rectal stump; does not require extensive mobilization of left colon, provides preservation of reservoir function of rectum, possibility of local access, which leads to significant reduction of risks of intra- and postoperative complications, reduced length of patient’s stay in surgical department, complexity of surgical treatment, making the operation reproducible.
