FIELD: medicine; cardiology; neurology.
SUBSTANCE: angiography of intra- and extracranial parts of carotid arteries is performed. An anatomical map of the vascular bed is constructed. Coronary conductor with J-shaped bend is brought. Straightening of the target carotid artery is confirmed by fluoroscopy. A distal cerebral protection system is installed. An anatomical map of the vascular bed is re-built for further navigation along the erected carotid artery followed by predilatation of stenotic segment of carotid artery and stent implantation. A distal cerebral protection system is removed, and control angiographic images of intra- and extracranial carotid arteries are performed.
EFFECT: method increases the efficiency of the surgical intervention, enables to straighten the tortuosity of the internal carotid artery and simplifies the establishment of the distal system of cerebral protection, reduces periprocedural complications, which will allow improving the patient’s quality and life expectancy due to preventive insertion of the coronary conductor into the target internal carotid artery.
