FIELD: medicine.
SUBSTANCE: invention relates to oncology, colorectal surgery. After dissection of transverse fascia in interfascial space, single access system is installed. Dissection is performed in cranial, medial and caudal directions, as well as separation of tissues with formation of retroperitoneal space and identification of ureter and gonadal vessels. Dissection is continued in the cranial and cranio-medial directions until the inferior horizontal branch of the duodenum is detected. That is followed by a pre-aortic dissection to reveal the base of the inferior mesenteric artery. Dissection is continued along the left common iliac artery in the proximal direction to the artery bifurcation and the hypogastric plexus identification above it, then along inner surface of left visceral nerve with displacement of preaortic fat towards removed part of mesentery. Inferior mesenteric artery is skeletonised and the vessels are selectively transected. Peritoneum is transected within the duodenum to communicate the retroperitoneal space with the abdominal cavity.
EFFECT: method provides the possibility of performing a minimally invasive surgical intervention in colon cancer in all categories of patients, including in patients with obesity and adhesions in the abdominal cavity, reduces operative injuries due to successive retroperitoneal dissection.
