SUBSTANCE: invention can be used for performing lymphadenectomy in case of uterine cancer, recurrent tumors of lumbar lymph nodes in case of ovarian cancer. First trocar is installed above the navel. Then trocars are installed in the left iliac region through all layers of the anterior abdominal wall to the peritoneum and in front of the peritoneum, unfolding the trocars to the psoas muscle. Second trocar with a stiletto is inserted above the anterior superior iliac spine, the third and fourth trocars are inserted above the second in the direction of the costal arch. Dissection of anatomical landmarks is performed—left common iliac artery, left ureter, gonadal vessels, left side of aorta, inferior mesenteric artery. Para-aortic lymph nodes are dissected along the lateral side, followed by aortocaval lymph nodes along the anterior wall of the aorta. Trocars are reinstalled, and right common iliac artery, right ureter, gonadal vessels, right side of aorta, inferior vena cava are separated. Paracaval lymph nodes are dissected.

EFFECT: method provides minimally invasive and adequate access to the removed group of lymph nodes, which allows performing the lymphadenectomy safely and fully, including in patients with short stature and obesity due to staged lymphadenectomy with trocar replacement.
