FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to ophthalmology. Tripartite intraocular lens is implanted during combined surgery of glaucoma and cataract. Phacoemulsification and non-penetrating deep sclerectomy are performed simultaneously. Main corneal incision, two paracenteses, capsulorhexis are performed, an intracapsular ring is implanted, cataract phacoemulsification is performed. Then three-part IOL is implanted, haptic elements are filled in iridociliary sulcus, optical portion is placed in a capsular sac, a non-penetrating deep sclerectomy is performed, a blade is used to form a rectangular superficial scleral flap with a base at limb measuring 4.0×4.0 mm and cut out by one third of thickness of sclera with entry into surface layers of cornea by 1.0 mm. Then the blade is used to form a rectangular deep scleral flap with a base at limb measuring 3.0×3.0 mm, it is cut out, leaving a layer of sclera under the flap, and Schlemm's canal is exposed, a portion of the ciliary body is exposed. After that deep scleral flap is excised together with outer wall of Schlemm's canal and strip of corneoscleral tissue up to Descemet's membrane with width of 1.0 mm, a juxtacanalicular layer with an endothelium is removed with an iris forceps from the surface of an internal wall of the Schlemm's canal; a scleral bed is filled with a Healaflow drainage implant; a superficial flap is laid; the conjunctiva is closed with interrupted sutures.
EFFECT: method provides long-term fixation of IOL in case of ligamentous apparatus weakness, prolongation of hypotensive effect and arrest of progression of glaucoma optic neuropathy.
