SUBSTANCE: invention relates to medicine, specifically to ophthalmology. For combined antiglaucomatous operation with lens extraction in 3.0 mm from limb with knife in width of 2.2 mm through conjunctiva in anterior-posterior direction, an intrascleral tunnel is formed in surface layers of sclera with outlet of knife into anterior chamber through its angle. Then - two limbal paracenteses with diameter of 1.0 mm. Anterior crystalline lens capsule is incised, circular capsulorhexis, hydration, crystalline hydrodelineation, crystalline lens fracture and aspiration. Aspiration-irrigation system is used to polish the posterior capsule of the crystalline lens. Intrascleral tunnel is expanded to 3 mm, through it an elastic intraocular lens is implanted into a capsular sac filled with a viscoelastic. Through pericorneal coloboma of the iris is formed. Deep flap of the base of the intrascleral tunnel is completely removed together with the trabecular apparatus. 4 mm xenoplast drainage is implanted into an intrascleral tunnel. Continuous suture 10/0 is applied on the conjunctive, paracentesises are hydrated.
EFFECT: method enables performing phacoemulsification, implantation of an intraocular lens through one scleral approach, forming a pericorneal coloboma, activating a uveoscleral fluid outflow, implanting a drainage, which leads to a prolonged hypotensive effect and a high visual effect.
