FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to cardiology and X-ray endovascular diagnostics and treatment. Guide catheter connected to a Y-connector in a coronary artery (CA) orifice is inserted, and selective coronary angiography (CG) is performed. If observing coronary artery thrombosis, the coronary conductor is delivered through the thrombosis to distal portions of the infarction-related coronary artery. Then, along the coronary guide, an extension guide catheter (EGC) is delivered through the well of the guide catheter to the proximal edge of the thrombus. Three-way valve of the Y-connector is connected to two 50 ml syringes. Negative pressure is created in the first syringe. Then the EGC is slowly advanced in the distal direction and returned to the initial position before the thrombosis. Manipulation is performed several times to stop the flow of thrombotic material or aspiration in total 200 ml of blood. When the first syringe is filled with aspirated blood by 80%, negative pressure is created in the second syringe and the Y-connector valve is switched to it, only then the filled syringe is removed. Empty syringe is connected to the free connection, and when the second syringe is filled, the switching procedure is repeated. Thereafter, the EGC is removed, and a control aspiration from the guide catheter is performed.
EFFECT: method enables to effectively aspirate thrombi from the coronary artery, preventing the development of the no-reflow phenomenon, does not require the use of specialized instruments, which significantly reduces the time from the beginning of the PCI procedure to the restoration of blood flow through the infarction-related coronary artery, is safe, can be used during PCI in coronary artery thrombosis in patients with ACS.
