FIELD: medicine.
SUBSTANCE: invention relates to traumatology and orthopedics and can be used for correction of rigid pronation contracture of forearm joints in children with spastic cerebral paralysis. Detorsion osteotomy of radial bone is performed from longitudinal approach along lateral surface of forearm to hypercorrection of forearm and hand. External osteosynthesis of a radial bone is performed with a plate. Square pronator is then approached from a medial approach of the lower one-third of the forearm. Myotenotomy of the square pronator is performed along the axis of the ulna; the square pronator is detached from the radial bone along the entire length. Ulnar flexor tendon is separated from the same approach, and its transposition is performed along the dorsal surface of the forearm through the formed canal of soft tissues to the radial bone in the distal direction from the osteotomy zone and the plate location. Tendon is fixed with an anchor, the wounds are closed, and the extremity is immobilized.
EFFECT: method provides correction of rigid pronation contracture of forearm joints, combined with torsional deformation of forearm bones by changing the position and attachment points of the square pronator muscle, the ulnar flexor tendon.
