FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to plastic surgery. Marking is made corresponding to the Weiss pattern and additionally marked area in the lower pole of the mammary gland, as shown in Fig. 1. De-epidermisation of the flap on the upper leg and the flap in the region of the lower pole of the mammary gland is carried out; at the cephalic edge of the de-epidermised flap of the lower pole, a semi-lunar breast tissue is incised, as shown in Fig. 5. Breast tissues are dissected to visualize the retromammary space and fibres of the pectoralis major muscle, the mammary gland is mobilized from the pectoralis major muscle, an incision is made perpendicular to the course of the pectoralis major muscle fibres, thus forming an approach to the subpectral intermuscular space. Further, a dissection and formation of a pocket is carried out, followed by installation of a silicone implant. Breast tissue is resected while preserving a superficial fascia; a de-epidermised flap of the lower pole is mobilized, thinned and laid on the surface of the implant. Then it is sutured to the pectoralis major muscle and superficial fascia to seal and isolate the plane with the installed implant. Breast tissues are sutured along the median line of the mammary gland, the areola is fixed to the skin according to the preoperative marking, then the breast tissues are closed in layers according to the marking.

EFFECT: method allowing to perform a T-shaped lift with endoprosthesis replacement with maximum preservation of fascial systems of mammary glands with preservation of the majority of vascular and nerve bundles, which, in turn, minimizes the risk of postoperative complications, such as haematoma, loss of sensitivity; provides less contouring of the implant and more natural tactile sensations from the breast; makes it possible to create more stable and dense pocket, which reduces prospect of implant displacement in early and late postoperative period, which significantly reduces probability of recurrent ptosis of mammary glands.
