FIELD: medicine.
SUBSTANCE: invention relates to anesthesiology, traumatology and orthopedics, and can be used in operations on lower extremities. One-stage high spinal anesthesia is performed by puncture of the spinal space through a paramedial approach at the level of L4–L5. Single introduction of isobaric solution of the preparation "BlokkoS" in dose of 20 mg with bubbling of 5 ml of cerebrospinal fluid is carried out. Blockade is controlled by tilting the head end of table to 45° under the control of non-invasive measurement of blood pressure and heart rate followed by intravenous drop introduction of ketamine in a subnarcotic dose of 0.7 mg/kg as part of an intraoperative volume of infusion therapy.
EFFECT: method enables increasing the anesthesia time by developing a high neuromuscular block to the level of the first-second thoracic vertebrae (Th1-2), as well as potentiating the analgesic effect of the spinal block and to anticipate the so-called "patient presence syndrome" with stability of central hemodynamics during the intervention and in the postoperative period due to combination of high spinal anesthesia with subnarcotic dose of ketamine by drip method.
