FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to abdominal surgery, and can be used in treating chronic pancreatitis (CP) complicated by duodenal obstruction. Main pancreatic duct is opened longitudinally along the whole length, concrements are removed and strictures are excised. Pancreatic head tissue is resected with preservation of parenchyma layer along inner edge of duodenum and preservation of branches of upper and lower pancreaticoduodenal arteries. Duodenotomy is performed with an incision of at least 5 cm. Small intestine loop excluded from digestion according to Roux is laid above the duodenum in an isoperistaltic direction, repeating its course. On the side of the small intestine loop wall facing the duodenum in a projection of the duodenotomic opening, a small intestinal lumen is opened with an incision of the same length as the duodenal incision. Side-to-side duodenoenteroanastomosis and a single-row longitudinal pancreatoenteroanastomosis are formed. Bile duct and pancreatic duct drains are taken out through the plugged end of the small intestine loop excluded from digestion according to Roux. Suspended enterostoma is formed.

EFFECT: method provides an increase in the effectiveness of the surgical treatment of CP complicated by duodenal obstruction, eliminating the main complications of CP by levelling the compression and its subsequent prevention, as well as maximum preservation of motor-evacuation and secretory function of duodenum and minimization of exocrine and endocrine pancreatic insufficiency.
