SUBSTANCE: invention relates to surgery and may be applicable for the surgical treatment of perforations of the posterior wall of the duodenum. Operation is carried out in two stages. At the first stage, a duodenotomy is performed, a major duodenal papilla and wound of the back wall of the duodenum are sutured, the common bile duct is crossed in the lower third, the pancreas is crossed in the area of the isthmus, the pancreatic duct of the distal and proximal pancreatic stump is drained by T-shaped single-channel drainage having two symmetric conical branches, the diameter of which decreases from the center to the ends by half, and which, after their immersion into the lumen of the duct, are fixed to the wall with a purse-string suture, the common bile duct is drained, bile and pancreatic juice are removed from the injured area and returned to the digestive system through a nasointestinal probe. At the second stage, the choledochus duodenal anastomosis of the “end to side” type and pancreaticofistulogastroanastomosis are formed.

EFFECT: method allows to reduce the risk of pancreatitis, water and electrolyte disorders, phlegmon and abdominal sepsis.
