FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to traumatology and orthopaedics, and can be used for surgical treatment of malignant new growths of femoral bone with application of endoprosthesis with individual shortened segment of cementless fixation. Before performing the surgical treatment, a spatial visualization of the affected femur is determined, and the state of the surrounding soft tissues, vascular and nerve structures is assessed. Skin and subcutaneous fat are incised on the patient's back along the anterolateral surface of the middle one-third of the thigh to a projection of the femoral condyles; a fascia lata is dissected, and a rectus muscle of thigh and a lateral vastus muscle of thigh are separated. Segment of the femur with a malignant growth is approached. Femoral osteotomy is performed in the proximal direction and distal to the malignant growth by 3 cm. Resected portion of the bone is removed. Using an individual impactor, an individual hollow shortened cementless leg of the endoprosthesis is hammered into the spongy bone of the remaining distal femur. Using the guide, the hollow shortened leg is blocked with 1-2 screws. Bone marrow canal of the proximal femoral bone is developed with the help of the reamers and the rasp to the required size. Standard cementless stem is implanted in a proximal part of a femur, and an endoprosthesis of the required length of reconstruction is assembled. Wound is washed, a silicone drainage is installed and the wound is closed in layers.
EFFECT: method provides reliable recovery of the functional and motor capabilities of the patient with malignant neoplasms of the femur, providing the conditions for maintaining the anatomical position of the femoral bones after surgical treatment, as well as improved quality of life of the patient due to the peculiarities of the method implementation.
