FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to traumatology and orthopedics, and can be used in the surgical treatment of patients with malignant neoplasm of the distal radius. Before surgical treatment, MSCT and MRI are performed to visualize the affected radius and surrounding soft tissues. Access is carried out along the dorsum of the forearm and structures that are not involved in the tumor are isolated. An individual resection block, previously made on the basis of MSCT data, is installed on the radius and 3 metacarpal bone, and osteotomy of the radius and bones of the proximal wrist is performed. The tumor is isolated from the remaining surrounding tissues and the preparation is removed in a single block. Next, the wrist component of the wrist joint endoprosthesis is installed with fixation with two cannulated screws and the implantation of the radial component of the wrist joint endoprosthesis is performed under the control of an image intensifier. Then, the radial and carpal components of the wrist joint endoprosthesis are fixed between themselves, holes are drilled in the distal part of the ulna at the level of the technological holes in the radial component of the endoprosthesis, and the ulna is fixed to the radial component of the wrist endoprosthesis with a wire fixator made of cerclage wire and lavsan.
EFFECT: method allows restoring the functional integrity and motor capabilities of the patient, to provide conditions for the prevention of dislocations in the wrist joint and the development of clubhand after surgical treatment, as well as a sufficient increase in the patient's quality of life due to the combination of techniques of the claimed invention.
