FIELD: medical science.
SUBSTANCE: invention refers to medicine, namely to traumatology and orthopaedics, and can be used for treating malignant new growths of the tibia using an endoprosthesis with shortened leg of cement fixation. Skin and subcutaneous fat are incised along an anterior surface of shin in a patient lying on his/her back. Muscles are dissected away from the tibia in stages from the medial and lateral sides, and the neoplasm is approached. Image intensifier assisted osteotomy of the tibia with the malignant new growth is performed. Resected portion of the shinbone is dissected away from the posterior group of muscles. Malignant new growth is removed en bloc. Intramedullary canal of the remaining distal portion of the tibia is developed to remove the contents of the intramedullary canal. Shortened leg of cement fixation is implanted. Guide is used to lock the short leg with 1-2 screws. Intramedullary canal of the proximal portion of the tibia is developed, and a cement-free leg is implanted into the proximal portion of the tibia. Endoprosthesis of the required repair length is assembled. Calf muscle is separated from the medial side with dissecting its medial head from a junction into an Achilles tendon. Separated muscle flap is rotated on the anterior surface of the shin, covered with the endoprosthesis and fixed to surrounding tissues, forming a muscular case for the endoprosthesis. Two silicone drainages with active aspiration are left: at the endoprosthesis and in the donor zone of the flap.
EFFECT: method provides functional and motor recovery, higher rehabilitation potential, as well as improved patient’s quality of life ensured by preserving the anatomic position of the bone after the surgical management.
