METHOD FOR TREATING PURULENT PANCREATITIS Russian patent published in 1996 - IPC

FIELD: medicine. SUBSTANCE: method involves dissecting in wide cut gastrocolic ligament, evacuating pyonecrotic mass from the pancreas region and parapancreatic fat, bringing drainage tubes to the head, body and tail of the pancreas and forming additional drainage canal isolated from the abdominal cavity made from the greater omentum between the retroperitoneal space and the inferior angle of the laparotomic wound. To do it, a 6x4 cm wide window is made in the central part of the greater omentum corresponding to the projection of the inferior boundary of the retroperitoneal fat lesion. The boundary of this window is sutured to the posterior parietal peritoneum in the region of the lower pole of the retroperitoneal fat lesion. The free edges of the greater omentum are sutured to the parietal peritoneum of the inferior corner of the laparotomic cut, a portion of the posterior parietal peritoneum restricted by the window is dissected and additional drainage tubes and rubber-gauze tampons are introduced into the retroperitoneal space through the formed drainage canal. Pancreas and retroperitoneal fat region are washed twice a day during the first two days in the postoperative period. Permanent infusion with 0.02% chlorohexidine aqueous solution is carried out at the third day in the amount of 2.0-2.5 l per day through tubes located near the head, body and tail of the pancreas until pus and sequesters stop discharging. EFFECT: enhanced effectiveness of washing and draining purulent lesions; prevented spread.
