METHOD FOR TREATING THE CASES OF PAINFUL CLOSED-ANGLE GLAUCOMA Russian patent published in 1999 - IPC

FIELD: medicine. SUBSTANCE: method involves forming round trepanation holes in sclera 2-7 mm far from the limb. Superficial U-shaped flap is cut in the inferior part of the eyeball before creating the trepanation holes. Flap base is turned to the limb passing through a peripheral area of cornea. The trepanation holes are created on both sides of the U-shaped flap, three on each side, like disk on leg being 1/5 times sclera thickness large. Two rectangular trepanation holes are produced near equatorial edge of the U-shaped flap edge. In creating one of them, sclera is retained on the internal side of the rectangle and implanted between the holes. Middle layers of sclera are notched near the limb and they are removed together with peripheral layers of the cornea to Descemet's membrane without opening the anterior chamber. Iris region having no blood vessels is selected and its coagulation is carried out through Descemet's membrane, in making cross-shaped cut in the Descemet's membrane and trabecule being 1.5 mm in size. Iridectomy is carried out with protruding iris apex being removed to create fistula into the posterior chamber. Collagenous tubular drain is sutured with one of its ends to the fistula. The scleral flap is fixed on its original place. EFFECT: enhanced effectiveness of hypotensive action; improved cosmetic properties. 5 dwg
