METHOD FOR PERFORMING PROSTATE ADENOMECTOMY Russian patent published in 2001 - IPC

FIELD: medicine. SUBSTANCE: method involves carrying out transvesicular access to the adenoma in making transverse incision on the anterior urinary bladder wall being 3-4 cm in size. Urinary bladder neck is cut with a knife making sure of full coagulation of the injured blood vessels. Posterior semicircular incision of the neck begins at 5 o'clock position and ends at 7 o'clock position of imaginary dial plate relative to the internal urethra orifice. The adenoma is removed step-by-step as discrete separated fragments. Catheter corresponding to urethra diameter is retained in the urethra in the period from the beginning of cutting urinary bladder neck tissues to the moment the urinary bladder neck wound is sutured. In suturing the urinary bladder neck, the whole detrusor thickness is sewn through. The first ligature is put near the external wound edge and thread is disposed immediately outside of the wound boundary. The neck wound edges are brought close by means of 2-3 sutures to the left and to the right from the internal urethra orifice. After the adenoma being removed and vesicourethral segment wound being sutured, adenoma bed is sutured with the same thread. The thread is tied with external and internal part of the surgical capsule being brought close to each other. Anterior urinary bladder wall wound is sealed with single row of sutures. The urinary bladder is drained by introducing draining tube into its cavity through a puncture in the anterior wall. EFFECT: enhanced effectiveness of treatment. 3 cl, 1 dwg
