FIELD: medical equipment and used in vascular surgery, proctology and other spheres of medicine, in which ultrasonic surgical effect is exerted on the blood vessel or cavernous body. SUBSTANCE: the ultrasonic instrument has a concentrator waveguide with a pointed end, by which a puncture of the wall of the vessel or cavernous body is accomplished, and a working part, which is introduced through this puncture inside the vessel or cavernous body thus realizing the method of ultrasonic action. The ultrasonic instrument is used in a system additionally having an ultrasonic generator, duct or ducts for movement of the liquid phase and a device for feeding and evacuation of the liquid phase with a reversible drive providing at a straight direction of motion an intake of the liquid phase through the duct or ducts for movement of the liquid phase, and at a reverse direction of motion the discharge of the liquid phase through the duct or ducts for movement of the liquid phase. EFFECT: provided action on the inner cavity of the vessel or cavernous body at a reduced loss of blood, reduced traumatism of tissues, eliminated necessity of use of devices cutting out the vessel from the blood flow. 48 cl, 13 dwg
