FIELD: medicine, orthopedics. SUBSTANCE: one should carry out arthroscopy due to standard anterior-external access. Defect of articular cartilage should be prepared by removing subchondral osseous plate, osseous-cartilaginous allotransplants should be prepared at necessary diameter and height by applying previously prepared material (treated with FDMP solution formalin-dimexide- monomycin-prednisolone), being perpendicular to articular condyloid surface, in defect area one should form canals in trabecular bone. Osseous-cartilaginous allotransplants should be successively installed into canals. Fixation of operated limb is not conducted. At applying combination of auto- and allotransplants, the former are taken out of the most favorable sites to take transplants (for example, medial femoral condyle of peripheral part in patellofemoral articulation). Transplants are installed into canals in checkered order that enables to perform therapy of full-layer defects in articular surface of any size at decreased traumaticity of operation that prevents detachment of transplants.
