METHOD FOR COVERING DUODENAL STUMP AT GASTRIC RESECTION Russian patent published in 2004 - IPC

FIELD: medicine, surgery.

SUBSTANCE: one should perform longitudinal pyloroduodenotomy, stop hemorrhage out of large chronic penetrating ulcer of posterior and median duodenal walls, mobilize antral gastric department by keeping nourishing vessels, dissect mucous-submucous layer of antral gastric department and initial duodenal part up to proximal ulcerous edge after longitudinal dissection of anterior wall of antral gastric department and its unfolding, form a tube as truncated cone out of serous-muscular flap due to overlapping and suturing the edges of serous-muscular flap by a "skirt"-type technique, apply purse-string suture onto tube's free edge, direct ligatures of purse-string suture into duodenal lumen to be withdrawn through pyloroduodenal incision outwards, carry out tubal invagination into duodenal lumen by tightening by ligatures, bind ligatures of purse-string suture, suture posterior tubal wall to ulcerous edges, suture duodenal wound in longitudinal direction by taking up anterior wall of invaginated serous-muscular cone into sutures. The method enables to achieve high strength and hermetic nature of duodenal stump and obtain quick healing of ulcer.
