SUBSTANCE: the present innovation deals with operations on coronary shunting. One should perform direct myocardial revascularization with free autoarterial transplant. At forming a transplant one should cross anterior tibial artery at the border of the middle and inferior third of patient's shin, cross dorsal artery of foot immediately above the site of its partition. This method enables to apply original transplant for direct myocardial revascularization at ischemic cardiac disease.
