FIELD: medicine, traumatology.
SUBSTANCE: one should perform longitudinal cutaneous incisions in area of ankles with subsequence access to tibial metaphysis and external ankle followed by development of the canal through tibial metaphyses. Additionally, about 1-2 cm above the first canal of 4-10 mm diameter developed through tibial metaphyses one should perform the second of less diameter - 3.5 mm to introduce a tightening bolt into it and with the help of a nut it is possible to tighten the fork of talocrural articulation at rear bending of patient's foot at the angle of 20° with the help of a doctor, then due to median access one should open patellar own ligament to isolate its longitudinal marginal part corresponding by its thickness to the diameter of the first canal, then one should isolate osseous fragments from adjacent tibial tuberosity and patella, one of them being equal by its thickness to that of the first canal, and another being 1-2 mm less, the edges of both fragments should be sutured with Dacron ligature by leaving their ends loose, then autotransplant developed that corresponds by its length to the first canal should be applied with its sharper end to the front to be applied in the canal as a newly developed ligament which should be tightened and fixed with the help of ligatures towards the ends of tightening bolt to provide the tension of the ligament applied, creates reliable stability of talocrural articulation without restricting the mobility of tibial bones.
EFFECT: higher efficiency of therapy.
