SUBSTANCE: invention relates to medicine, namely to traumatology and orthopedics, and can be used to treat patients with chronic injuries of the distal tibiofibular syndesmosis. After removal of the scar tissue between the fibular notch of the tibia and the distal fibula, narrowing of the extended fork of the ankle joint is performed using a bone cap and fluoroscopic control is performed. Then, the syndesmosis is fixed with a positioning cancellous screw with a diameter of 4.0 mm with a partial thread, which is passed through the hole in the plate fixing the fibula, through the fibula and two cortical layers of the tibia 3 cm above and parallel to the joint space of the ankle joint at an angle of 30 degrees to the frontal plane from the outside behind the inside and anteriorly. Additionally, the distal tibiofibular syndesmosis is fixed with a button lock, which is passed through the hole in the plate, 2 cm above the ankle joint, repeating the direction of the positioning screw. 2.5 months after the operation, the position screw is removed, while maintaining the dynamic fixator.

EFFECT: method provides improved treatment results by restoring the anatomy of the tibiofibular articulation and fork of the ankle joint due to the correct positioning of the fibula in the notch of the tibia.
