SUBSTANCE: the present innovation deals with operative treatment of prostatic malignant tumor. One should carry out retropubic prostatectomy, apply radial urethral sutures, suture posterior urethral semicircumference twice with nonabsorbable monolithic ligature at the level of 5 and 7 o'clock of hypothetical clock face not suturing urethral mucosa. Then, one should suture the cervix of urinary bladder with anastomosis ligatures. Due to tightening up the ligatures one should form a cysto-urethral anastomosis. The ends of monolithic ligature should be withdrawn onto anterior abdominal wall, one should suture operational wound layer-by-layer. Provisionally, the ends of monolithic ligature should be bound above aponeurosis after layer-by-layer suturing the operational wound. On the 12^th-14^th d after operation it is necessary to remove urethral catheter and perform urethroscopy. Under vision control, one should tighten up ligatures so, that urethra lumen looks like a cross-sectional fissure. Then one should clinically evaluate urinary continence, end of ligature should be cut off above the knot. The innovation provides prophylaxis of urinary incontinence after radical prostatectomy.
