SURGICAL METHOD FOR SUBSTITUTING PENETRATING DEFECT OF LOWER EYELID Russian patent published in 2006 - IPC A61F9/07 A61B17/03 

FIELD: medicine.

SUBSTANCE: method involves making incision towards nasolabial fold and further downward along the nasolabial fold. Skin flap is separated as wide strip. The conjunctiva defect is compensated by redistributing it from the lower conjunctival fornix and from eyeball. Tarsal eyelid plate defect is substituted with free transplant taken from native ear cartilage. It is attached to tarsal plate remnant or to periosteum in the area of internal and/or external angle of lower eyelid. The skin flap is rotated over the defect area. U-shaped sutures are placed by penetrating all layers of newly created lower eyelid for building lower conjunctival fornix. The displaced skin flap is fixed with interrupted sutures along wound mouth and allover its whole area through elastic flexible compression plates fabricated from oligocarbonate methacrylate or polymethyl methacrylate. The plates are either hexagonal or rectangular and/or square with rounded angles. Round, hexagonal, oval or elliptical holes are available allover plate surface. Skin and mucous membrane are sutured above cartilaginous transplant with interrupted sutures. The sutures and plates are taken off from the wound step-by-step at the fifth-eighth day after the operation.

EFFECT: full compensation of defect; improved postoperative cicatrix quality; excluded skin flap slipping and shrinking.
