FIELD: medicine, vascular surgery.

SUBSTANCE: the present innovation deals with carrying out vascular plasty upon arteries of inferior limbs in case of prolonged occlusion under condition of no large subcutaneous vein or other autotransplant. For this purpose, on should perform longitudinal endarter- and thrombectomy at the length up to 40 cm at keeping all functional collaterals at the whole distance of surface femoral artery at the background of heparin application. One should suture in an autovenous patch into distal department of arteriotomic foramen In pre-operational period it is necessary to introduce low-molecular heparins. Then one should conduct endarterthrombectomy at applying a single-row suture upon artery with atraumatic monoligature. In post-operational period one should introduce low-molecular heparins for about 10-14 d. The innovation enables to prevent post-operational lymphorrhea, save the present vein, decrease the time of squeezing the main circulation due to applying one row of sutures upon an artery, the absence of long patch, liquidate compression of collaterals and femoral artery with dissected anterior wall of the canal of adducting muscles and keep all functioning collaterals.

EFFECT: higher efficiency of arterial plasty.
