METHOD FOR OPERATIVE TREATMENT OF TENDINOUS RUPTURE OF SUPRASPINOUS MUSCLE Russian patent published in 2006 - IPC A61B17/56 

FIELD: medicine, orthopedics, traumatology.

SUBSTANCE: the present innovation deals with reconstructing the integrity of tendinous rupture of supraspinous muscle due to mobilizing and fixing the ruptured tendinous end with the help of an implant to humerus. It is necessary to form a receiving osseous channel beforehand in the site of natural fixation of supraspinous muscle's tendon, moreover, one should remove the part of cortical layer of large tubercle of humerus and form a conical hole of 5 mm depth. Then, in caput humerus it is necessary to form two parallel canals coinciding with implants' pull vector, for example, as ligatures, one should apply ligatures into the canals, their ends should be withdrawn onto the skin to be fixed in apparatus for external fixation applied at the level of middle third of humerus. After operation one should carry out tendinous distraction at the rate of 1 mm daily which should be finished at coincidence of roentgenocontrast mark placed at tendinous end, into receiving osseous channel at caput humerus. The system should be stabilized for 3 d, then apparatus should be dismantled, the ends of ligatures should be tightened into a knot to be placed at humerus periosteum that provides substitution of the defect and tendinous refixation of supraspinous muscle in case of chronic lesions due to own tissues not applying artificial implants or tendinous transposition of other muscles of humerus rotating cuff and, thus, atraumatically reconstruct the function of brachial joint.

EFFECT: higher efficiency of therapy.
