FIELD: medicine, gastroenterology.
SUBSTANCE: one should carry out esophageal manometry and measure average respiratory pressure in area of lower esophageal sphincter with a water-perfusion catheter for registering esophageal motor system. Additionally, one should simultaneously perform antroduodenal manometry. Under US control with a convex sensor at 3.5 MHz one should apply a water-perfusion 6-canal catheter for registering antroduodenal motor system so, that two its canals be situated in the stomach, one - in pyloric area and three - in duodenum, and two canals of a water-perfusion 8-canal catheter for registering esophageal motor system should be applied in area of lower esophageal sphincter. One should register the phases of hungry migrating motor complex of antroduodenal area and alteration of average respiratory pressure in area of lower esophageal sphincter during each phase. At the increase of average respiratory pressure in area of lower esophageal sphincter up to 14-34 mm mercury column during phases 2 and 3 against initial one being about 2-13 mm mercury column during phase 1 one should diagnose low risk of the development of gastroesophageal reflux disease. At the decrease of average respiratory pressure in area of lower esophageal sphincter up to 2-13 mm mercury column during phases 2 and 3 against initial one being about 14-34 mm mercury column during phase 1 one should diagnose average risk for the development of gastroesophageal reflux disease, at the absence of increased pressure up to 14-34 mm mercury column during phases 2 and 3 against initial one being about 2-13 mm mercury column during phase 1 it is possible to diagnose high risk for the development of gastroesophageal reflux disease. At no decreased pressure up to 2-13 mm mercury column during phases 2 and 3 against initial one being about 14-34 mm mercury column during phase 1 one should diagnose no risk for the development of gastroesophageal reflux disease. The innovation enables to conduct prophylactic measures in a more differentiated way.
EFFECT: higher accuracy of detection.
