FIELD: medicine.
SUBSTANCE: method involves applying local anesthesia by introducing 1 ml of 2% of lidocaine solution in subtennon space. Conjunctival flap and superficial rectangular scleral flap 4х4 mm are separated to 1/3 of sclera thickness, with the base turned towards limb. Cyclodialysis is formed at the top of triangular scleral flap to ciliary body. Triangular flap is cut out from deep sclera layers being 1/3 of its thickness large with the base turned to limb and removed together with external Sclemm's canal wall and adjacent corneal tissue up to descemet membrane. Endothelial lining of the Sclemm's canal is removed. Biological porous collagen drainage is implanted into prepared bed turned with one end towards Sclemm's canal, and its other end should project 1 mm outside from under superficial scleral flap. Narrow 2х4 mm large sclera strip being 1/2 of sclera thickness thick, is additionally cut out from one or other side of superficial scleral flap with its smaller part turned towards limb. Deep 2х2 mm large scleral flap is removed in Sclemm's canal projection zone. Intervascular iridotomy is carried out. The second biological porous collagen drainage is implanted, one end of which is introduced into the anterior chamber, and other one attached to deep sclera layers. Narrow scleral strip is returned to its place and fixed with a suture. Superficial scleral flap is returned to its place and fixed with two sutures. Both drainage members are manufactured from material containing bone collagen, taken from spongy bone of agricultural animals and saturated with sulfated bone glycosamines, as 0.8 mm thick, 1,5 mm wide and 4,0 mm long plates.
EFFECT: enhanced effectiveness of treatment; prolonged intraocular pressure stabilization; active intraocular liquid discharge via primary path under conjunctiva and via additional uveoscleral path with special porous biological collagenous drainage being used; excluded hemorrhages into anterior chamber and expulsive hemorrhages; reduced risk of traumatic complications.
