FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to ophthalmology. At the first stage, the conjunctiva is incised at the limb, the conjunctiva and Tenon's membrane are separated from the sclera, bipolar diathermocoagulation of the surface vessels of the sclera, a surface scleral flap of size 4 × 4 mm is cut out base to limb. Further, the middle layers of the sclera are removed with size of 2 × 4 mm to transparent part of cornea with opening of Schlemm's canal (SC) and removing them. Further, the endothelial lining of the SC is removed, a scleral bridge sized 1 × 4 mm and reverse cyclodialysis is carried out under the bridge and behind it with the help of a spatula. Minimum scleral tissue is left on top of the ciliary body in the anterior scleral bed. Implantation of collagen drainage under bridge in suprauveal space, then scleral flap size 1 × 4 mm behind the bridge over the entire thickness of the sclera. Further, the superficial scleral flap and conjunctiva are closed with interrupted sutures. At the second stage, YAG laser trabeculotomy is performed as follows: one hole is created in the trabecular apparatus in the area of the first stage of this operation with the help of a YAG laser and a single-mirror goniolens.

EFFECT: method provides activation of the trabecular and uveoscleral outflow tracts of the intraocular fluid for a long period of time, as well as filtration of the intraocular fluid into the subconjunctival space, achieving a persistent hypotensive effect, reducing the number of antihypertensive agents, reduced number of intra- and postoperative complications, reduced length of staying in hospital and possibility of delayed YAG-laser trabeculopuncture.
