FIELD: medicine, neurosurgery.
SUBSTANCE: one should carry out dopplerographic testing of cerebral vessels in dynamics both before and after therapy along with detection the index of peripheral resistance (IPR), vasoconstrictor reserve (R(-)) to hypocapnic loading and vasodilation reserve (R(+)) to hypercapnic loading. Moreover, additionally one should calculate cerebral autonomic index (CAI) by the following formula: CAI=R(-):R(+):IPR, according to CAI values of all arteries both before and after conservative and/or surgical therapy the result of therapy should be evaluated as efficient or doubtful or inefficient. If one should apply pulsation index as IPR, as R(-) - the coefficient of reactivity to hypocapnic loading, as R(+) - the coefficient of reactivity to hypercapnic loading, in that case if after therapy in all arteries CAI<0.27 or CAI>0.53 - the result of therapy should be evaluated as efficient; if CAI<0.27 and CAI>0.53 and/or in at least one artery the following value 0.27≤CAI<0.3 or 0.5<CAI≤0.53 is kept - the result should be evaluated as doubtful; if the following value 0.3≤CAI≤0.5 is either kept or appears in at least one artery - the result is considered to be inefficient. Additionally, in case of surgical therapy: the result should be evaluated as efficient, if CAI alterations towards the decrease or the increase are opposite to IPR alterations and analogous in all arteries, and if the analogy of such alterations is broken in at least one artery the evaluation is doubtful. The innovation suggested enables to evaluate true state of epileptic process in earlier terms of therapy and in a single digital format.
EFFECT: higher accuracy of evaluation.
