FIELD: medicine, surgery.

SUBSTANCE: the present innovation deals with surgical therapy of diabetes mellitus in transplantological centers and in specialized medical establishments. One should sample distal segment of pancreas in a donor to carry out its conservation. In the course of transplantation a pancreatic transplant for a recipient should be reversed and applied with its posterior surface to the front. Venous anastomosis with inferior mesenteric vein should be formed at the level of inferior horizontal duodenal branch. Arterial revascularization of the transplant should be fulfilled due to anastomosing a proximal end of the transplant's splenic artery with a recipient's aorta according to "end-to-side" type. Moreover, a distal end of splenic artery should be ligated. The innovation helps to provide the transplant with horizontal physiological drainage disposition, provides the chance to form anastomosis with proximal departments of small intestine, keep the circulation along the system of inferior mesenteric artery and exclude intersection of vessels at forming vascular anastomoses.
