FIELD: medicine, surgery, transplantation.
SUBSTANCE: the present innovation could be applied in treating insulin-resistant diabetes mellitus. Pancreatic transplant should be reversed and applied with its posterior surface to the front. One should form venous anastomosis of inferior mesenteric vein with transplant's splenic vein at the level of horizontal duodenal branch. Arterial revascularization of transplant should be carried out due to anastomosing distal end of splenic artery and a recipient's left communis iliac artery by "end-to-side" type and transplant's proximal end of splenic artery and aorta. The innovation favors transarterial shunting the excess of arterial blood and, thus, considerably decreases acute inflammatory and reperfusion phenomena in pancreatic transplant after its revascularization, there is the decrease in the risk of the development of transplant's acute pancreatitis, its destruction, the failure of pancreatoentero- or cystoanastomoses, it enables to improve the values of transplant's functioning in post-surgical period, the risk of acute disorder in circulation in left half of large intestine has been leveled, moreover, it is possible to achieve maximally high localization of the transplant applied. Moreover, the innovation in question enables to adequately cover the anastomosis with sigmoid flexure mesentery and isolate it against surrounding tissues that leads to decreased risk of post-surgical complications and accelerates pancreatic reinnervation.
