FIELD: medicine; ophthalmosurgery.
SUBSTANCE: glaucoma surgery zone is scanned by ultrasound in the sagittal plane to see if restoration of the outflow tracts in the zone is possible. If a medium or large intra-scleral cavity is found, then the cataract is first removed through a tunnel dissection in the cornea and an intra-ocular lens is implanted. Then the micro-tunnel access is formed in the cornea and if visco-preparation is preserved in the anterior chamber, the tracts of outflow of intra-ocular fluid are restored. If a small intra-scleral cavity is found, then microinvasive deep sclerectomy is performed in the unscarred area. After then a tunnel dissection is made in cornea and the cataract is removed. Conjunctiva is stitched in the area of new surgery after the cataract surgery is finished and filtration of the intra-ocular fluid through the entocornea is checked.
EFFECT: surgery is shorter and less damaging; hypotension effect is sustainable, vision is better, patient rehabilitation period is shorter.
