METHOD OF SURGICAL TREATMENT OF ABDOMINAL AORTIC ANEURYSM Russian patent published in 2008 - IPC A61B17/00

SUBSTANCE: walls of aneurysm are strengthened along its length from top to bottom using a synthetic fabric and creating a reinforcing carcass in the form of strips around the aneurysm, similar to its lateral dimensions. Preliminarily, a series of holes for neo angiogenesis saving rigidity of the strips is made, the strips being laid consequently one under another. The intraoperative sonographic evaluation of inner and outer diameters, aneurysm wall thickness is performed, as well as the control of the following bloodstream parameters: speed, turbulence, laminarity. Then, using the ultrasound duplex control, gradual constriction and fixation of ends of the strips in the direction from the upper pole of aneurysm to the bottom one is performed till the detection of normalisation in bloodstream speed and type close to laminar, and till obtaining inner diameter of aneurysm opening corresponding to diameter of aneurysm opening on its unexpanded part, detachment of thrombotic masses in the cup of aneurysm being excluded. Then the strips are sewed against each other. Also, if the aneurysm spreads to the area of bifurcation of aorta, both side arteries are additionally covered. The diameter of the perforated holes on the strips is limited to 1.5-5 mm.

EFFECT: possibility to perform surgical treatment taking into account bloodstream indicants and stimulate angiogenesis during postsurgical period.
