METHOD OF MASTOPEXY Russian patent published in 2008 - IPC A61B17/00 

FIELD: medicine; surgery.

SUBSTANCE: during operation form an open, put axes of a mammary gland, lines of submammary plica. Define a new contour auras at location of an upper edge of an areola as finger projection to a forward surface of the breast at its placement in the submamary plica on a line of an axis of a mammary gland - (A) point, drawing round a contour papilla mammillary-areolar complex in diameter of 4.5-5.0 cm, place a (B) point on a mammary gland axis on 2 cm downwards from a bottom edge of an areola; draw an ellipse through the received points (A) and (B) defining the external borders of deepidermisation, divide the line of the submamary plica into four equal parts, place (C) and (D) points along the edges of two internal quarters, and the border between them passes through a mammary gland axis; perform deepidermisation around the mammillary-areolar complex; after that carry out dissection of skin and hypodermic fat on periphery from the deepidermisation zone in a direction to the top quadrants of the mammary gland to the level of the second intercostal space, 3 cm in a medial and lateral sides and down to the submammary plica, then put the fixing stitches which are passing through an upper edge of the deepidermised skin, an upper edge of own tissue of a gland and a fascia of pectoral muscles, put in stitches in a point (E) - in a place of crossing of a line of of the mammary gland axis and the second intercostal space, and points (F), (G), located from the both sides from the point (E) at distance of 3 cm along the second intercostal space, dissect own tissue of the mammary gland along the (C) (B) (D) line in the bottom quadrants of the mammary gland, forming a flap on the basis (C) (D); form a pocket from the flap basis (C) (D) in the retromammary area upwards to the third intercostal space, immerse the flap in the pocket and fix three stitches which are passing through an upper edge of the flap of own tissue of the gland and a fascia of pectoral muscles in a point (H) - in a place of crossing of an axis of a mammary gland and the third intercostal space, in points (I) and (J), located along the both sides from the point (H) on distance of 2 cm on along the third intercostal space, the top part of own tissue of a gland in the area of a cut (C) (B) (D) fix with separate stitches to the twisted flap, fix papilla upper edge with a separate stitch in the (A) point, then stitch the skin round the papilla using a purse-string intradermal suture.

EFFECT: method allows to reduce the sizes of a dermal cut and to provide new points of fixation; to bring risk of relapse of a ptosis to naught and to reduce tension of dermal sutures.
