METHOD FOR SURGICAL CORRECTING MASTOPTOSIS OUT OF PERIAREOLAR ACCESS Russian patent published in 2004 - IPC

SUBSTANCE: the present innovation deals with cutting, applying mammary axis, indicating new areolar contour, carrying out deepidermization around mammilary-areolar complex followed by applying fixing sutures coming through the upper edge of deepidermized skin, the upper edge of own glandular tissue and fascia of thoracic muscles in certain sequence. Moreover, after fulfilling deepidermization one should dissect skin and subcutaneous fiber along concentric mammary perimeter. Then one should fix mammary tissue to anterior thoracic wall in new position in several symmetric points and alteration of mammary configuration should be carried out with a crimping suture spyrally lifting from mammary bottom towards areolar edge. Then one should perform additional intraoperational marking by dividing areolar edge and that of cutaneous incision with marks for several symmetrical sectors. One should suture in the edges of deepidermization area due to applying reciprocal circular crimping dermal suture beginning and finishing in the upper mark along the median mammary line, moreover, the capture of areolar edge and its combination with the edge of cutaneous fragment should be performed, also, in several symmetrical points corresponding to the lines of marking for sectors by alternating suturing along the arches of marked sectors with successive application of purse-string suture that fixes cutaneous edge to areolar circle. The method enables to increase cosmetic effect and keep the function of glandular tissue.
