METHOD OF SURGICAL TREATMENT OF PES PLANO-VALGUS IN CHILDREN WITH INFANTILE CEREBRAL PARALYSIS Russian patent published in 2009 - IPC A61B17/56

SUBSTANCE: intake of bone autotransplant from lower one-third of fibular bone is carried out with simultaneous separation and transfer of long peronial muscle tendon from one cut. Inner capsulotomia of talar-navicular and talar-calcaneal joints is carried out. In case of ecvinus position of calcaneal bone Strier operation or one of achilloplasty types is performed. Longitudinal arch is restored and its valgus deformation is eliminated. Fixation in obtained normo-correction position with horizontally placed bone autotransplant, introduced between released from cartilage covering joint surfaces of talar and calcaneal bones, is carried out. Transosseous metalo-osteosynthesis is performed with Kirschner's wire, drawn in vertical direction through calcaneal bone, bone autotransplant, talar bone, distal tibia epimetaphysis. Tendon of long peronial muscle, transected in area of foot external side, is hypodermically transferred on talar bone neck. Tendon of long peronial muscle is passed through hole, made in talar bone neck and fixed with loop-shaped tension. Z-shaped shortening plasty of posterior tibial muscle is performed.

EFFECT: reliability of talar bone fixation in correct position, restoration of supporting function of foot, reduction of traumacity, reduction of post-operation complications, shortening of treatment terms.
