FIELD: medicine; ophthalmology.
SUBSTANCE: through corneal paracentesis performed, viscoelastic agent is introduced under iris within prospected operative area. Conjunctival and surface scleral grafts are formed. Scleral mid-layers in size 3×3 mm are excised. Then within projection of bottom of ciliary groove, deep scleral layers as a strip 2.5×1.5 mm concentric to limb are removed. The bared part of ciliary apparatus is cauterised. Posterior chamber is opened with an incision 1-2.5 mm being concentric to limb. Hydrogel drainage is inserted into posterior chamber at depth 2 mm. An external end of the drainage is placed on the bottom of scleral bed at exit end 1.5-2 mm. Surface scleral graft is placed back and fixed at the corners with two interrupted sutures. The external end of the drainage is fixed to episclera with one interrupted suture. Viscoelastic is washed out from the eye through paracentesis. Uninterrupted suture is applied on conjunctiva.
EFFECT: prevention of sudden IOP drop when posterior chamber is open, and ensured prolonged IOP balance in long-term postoperative period.
