FIELD: medicine.
SUBSTANCE: conjunctival flap and superficial scleral flap are cut out. Rectangular flap is removed from middle layers of sclera with outer wall of channel of Schlemm being removed and limbal edge of Descemet's membrane being naked. Deeper scleral layers are removed until inner layers of cornea-scleral part of trabecula being naked. Superficial scleral flap is fixed on angles and centres of flap sides by four interrupted sutures. Then viscous elastic agent is introduced under the fixed scleral flap. After that trabeculotomy is performed with knife "lance", the knife being passed under the fixed scleral flap towards its base. Drainage made of hydrophilic cross-linked poly-hydroxiethylmetacrylate-based material which is produced by polymerisation with immobilised covalently bonded medicinal preparations is implanted under sewn-on scleral flap. Preparations make anti-inflammatory effect and prevent development of commissural obliteration of formed outflow tract. Drainage can be made as a film or gel.
EFFECT: local commissural reaction to be prevented, efficiency and safety of method for secondary open angle glaucoma surgery to be increased, IOP to be normalised persistently.
